Patient Forms

Once you have completed the forms, call our office to schedule your first appointment. Bring the forms with you.

FORM 1: Patient Information Form
FORM 2: Dental and Medical History Form

Both forms are Adobe Acrobat files. You must have the Acrobat Reader on your computer in order to open and print Acrobat files. If you don’t have the Acrobat Reader, you can download it for free by clicking here.

Once you have downloaded and installed the Acrobat Reader, right-click on each of the below links, and select “Save Target As…”. Download each file to a location of your choice on your computer (the Desktop is always a good option). Once the download is complete, find the location where you saved the files. Double-clicking each file will open the form in the Acrobat Reader. To print each of the two forms, simply click the printer button.
