First, please allow us to welcome you to our office. We hope to make your visit as pleasant as possible.

This form explains to all of our patients the billing process of this office. The primary goal of our dental practice is to provide the highest quality oral health care in the most gentle, efficient and enthusiastic manner.  

Unfortunately, apart from the emotional and physical impact of any dental treatment, there is often a degree of financial impact as well. We would like to ease your potential financial burden as much as possible.

Your review of our financial guidelines will help greatly to avoid potential future misunderstandings and make it easier in future to allow us to continue to provide our best service.

Please understand that your dental insurance benefit program is a contract between you, your employer, and your insurance carrier. We are not a party to that contract. You (not your insurance carrier) are ultimately responsible for payment of services. Not all services are covered benefits in all contracts. Your employer has selected the level of coverage based on the premium paid.

Payment is expected at the time of service for all procedures.

If you would like us to bill your insurance, we need accurate personal and insurance information. You may be asked to provide your social security number and copies of your driver’s license and insurance card.

We will diagnose dental problems as accurately as possible, but changes may occur after the treatment is started. Based on our diagnosis, we will estimate the total cost prior to performing treatment.

We submit insurance claims as a patient courtesy, however that even though the contract exists between the patient/insured and the insurance company. We try to help all patients receive the maximum benefits their plans will allow for the treatment they need. Our business is providing excellent dental care. The insurance industry has made it nearly impossible for patients to file their own claims. When or if there are insurance difficulties, please know that we are working on your behalf and we pledge to do our best. In most cases you have authorized insurance payments to come directly to us. We will estimate your portion based on historical information from your insurance company. Your portion is due the day of service. Please be prepared to pay your estimated patient  portion.

Appointment Policy: Your dental appointments are scheduled carefully. Time, trained personnel and dental equipment are reserved for each procedure. Missed appointments add to the cost of dental care when reserved facilities are left waiting empty. In the event of repeated last minute cancellations (less than 24 hours) or no shows, you will be charged $25.00 before you can reschedule your next appointment.
